Work Environment

Your work will primarily take place in the homes of our patients, providing an intimate and personalized setting where you can truly connect with and understand their individual needs. Working from Monday to Friday within a structured 40-hour schedule, you'll have the freedom to create therapeutic relationships while delivering clinical care. Key Responsibilities

• Conduct thorough evaluations of patients to establish effective treatment plans based on physician referrals and individual needs.
• Implement tailored treatment programs focusing on rehabilitation and functional improvement while conducting progress assessments regularly.
• Utilize evidence-based practices to address diverse patient needs, delivering customized interventions that span orthopedic, pediatric, neurologic, and sports medicine specialties.
• Provide education and training to patients and their caregivers, ensuring they comprehend their condition and treatment plans, which may involve home exercise programs and preventive strategies.
• Demonstrate remarkable customer service, fostering trust and a positive rapport with patients to enhance their experience.
• Collaborate closely with a multidisciplinary team to maximize patient recovery and rehabilitation outcomes, sharing best practices and insights.
• Achieve and maintain minimum utilization goals, contributing to the overall success and business objectives of Evince Physical Therapy.

Patient Population / Client Focus

The patients you will serve range from children to seniors, requiring a variety of therapies tailored to their unique clinical needs. You will predominantly engage with individuals recovering from surgeries, managing chronic conditions, or experiencing functional limitations. By working in their homes, you will identify and address the challenges they face in their daily environments, fostering meaningful rehabilitation journeys.

Qualifications

• Minimum of a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active State License as a Doctor of Physical Therapist (DPT) issued by the Physical Therapy Examining Committee.
• At least two (2) years of clinical experience within a physical therapy setting.
• Current CPR certification to ensure patient safety during all therapeutic interactions.
• Strong interpersonal communication skills, both written and verbal, to maintain clear dialogue with patients and caregivers.
• Proficient in technology, including word processing and electronic health record systems.
• Ability to develop genuine relationships with patients and contribute positively to their recovery experiences.

Preferred Experience

Experience in home health physical therapy is preferred, as is familiarity with evidence-based modalities and rehabilitation technologies.
