Work Environment

This is a full-time, onsite position primarily focused on home care, which allows you to work directly with patients in their residences. You will typically work 40 hours per week with variable weekends as per patient needs. Your home visits will involve assessing and implementing treatment plans, while providing continuous support and guidance to patients and their families.

Key Responsibilities

• Conduct comprehensive evaluations and assessments of patients' physical conditions, including their functional limitations.
• Develop individualized treatment plans that address each patient's unique needs, ensuring they receive the best possible care.
• Implement and monitor treatment interventions, consistently re-evaluating progress based on patient feedback and outcomes.
• Maintain detailed, accurate documentation of patient treatments and progress through both electronic and written communication.
• Collaborate effectively with interdisciplinary team members, healthcare providers, and family members to support the patient's continuum of care.
• Educate patients and their families on rehabilitation exercises, home care strategies, and wellness practices to promote ongoing recovery.
• Participate in community outreach efforts and quality improvement initiatives to enhance service delivery within the organization.
• Ensure compliance with all health regulations, professional standards, and state licensing requirements.

Patient Population / Client Focus

Your primary focus will be on patients with orthopedic conditions, post-surgical needs, and progressive disabilities requiring rehabilitation. You will empower individuals of varied ages to regain independence and improve their quality of life in their familiar home environments. As you work closely with patients, you will foster meaningful relationships that support their physical and emotional recovery.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current state license in Physical Therapy is necessary to ensure compliance with local regulations.
• Basic Life Support (BLS) certification is preferred to ensure patient safety in emergency situations.
• Strong verbal and written communication skills to effectively collaborate with patients, families, and healthcare teams.
• Proficiency in word processing and familiarity with electronic health record (EHR) systems.

Preferred Experience

A minimum of two years of experience in a similar physical therapy role is required, with a preference for experience working in home healthcare or rehabilitation settings. Compensation and Benefits

The expected salary range for this position is USD 100,000 – 120,000 annually, reflecting competitive compensation for your expertise and dedication.
