Swift Health Physical Therapy is seeking a dedicated and compassionate Full-Time Physical Therapist to join our rehabilitation facility in Keyport, New Jersey. Key Responsibilities

• Conduct comprehensive evaluations of patients, thoroughly reviewing their medical history and performance assessments to create individualized treatment plans.
• Communicate clearly with patients and their families regarding evaluation findings, educating them about the proposed treatment plans and actively involving them in setting achievable rehabilitation goals.
• Utilize evidence-based treatment protocols tailored to each patient's unique presentation and health care needs, adapting strategies as needed to enhance the patient's recovery journey.
• Collaborate with physicians and other healthcare providers to ensure integrated care; monitor patient progress and document any changes in their condition, necessitating adjustments to the plan of care.
• Develop discharge plans and follow-up care to ensure comprehensive and continued rehabilitation post-treatment; maintain accurate documentation of all patient interactions, including initial evaluations, progress notes, and discharge summaries.
• Manage time efficiently to meet productivity expectations while ensuring thorough patient documentation and engagement in hospital and community outreach initiatives.
• Proactively participate in departmental goal-setting and compliance with regulatory standards, ensuring the highest quality of care is consistently delivered.
• Commit to ongoing professional development by pursuing continuing education opportunities and sharing knowledge with peers; may also serve as a clinical instructor for interns or students.
• Champion customer experience initiatives, ensuring access to care is streamlined and adapting to patient needs throughout their rehabilitation process.

Patient Population / Client Focus

You will work with a diverse clientele, including individuals recovering from orthopedic surgeries, sports injuries, neurological conditions, and chronic illnesses. Your ability to identify and adapt to the unique needs of each patient will directly influence their recovery journey. Qualifications

• A Bachelor's Degree in Physical Therapy or a Doctorate degree in Physical Therapy (DPT) is required.
• A valid physical therapy license in the State of New Jersey is essential.
• Current American Heart Association BLS for Healthcare Providers CPR certification.
• Strong communication skills and a passion for helping others are critical for success in this role.

Preferred Experience

Compensation and Benefits
