Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient histories, conducting assessments, and recommending treatment plans that align with patient needs.
• Clearly communicate evaluation findings and treatment plans to patients and their families, involving them in goal-setting for their rehabilitation.
• Implement tailored treatment protocols based on evaluation findings and patient presentations, demonstrating adaptability to evolving healthcare practices and technologies.
• Collaborate with other healthcare professionals to ensure coordinated care. Maintain up-to-date documentation of patients' conditions and progress while recognizing when to modify treatment plans as needed.
• Develop detailed discharge plans and provide necessary follow-up care to ensure continuity of services.
• Manage time effectively, meeting productivity expectations while maintaining punctuality for meetings and therapy sessions.
• Participate in marketing activities to promote our services within the community and contribute to the achievement of departmental goals and objectives.
• Engage in ongoing professional development by attending workshops, pursuing continuing education opportunities, and taking on the role of clinical instructor for students.
• Facilitate patient access to care by actively participating in initiatives to remove barriers to treatment and enhance customer experience through flexibility.

Patient Population / Client Focus

As a Physical Therapist at Swift Health, you will work primarily with outpatient clients who present a variety of musculoskeletal issues, post-operative rehabilitation needs, and chronic pain conditions. Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution is required.
• A valid Physical Therapy license in the State of New Jersey is mandatory for this position.

Preferred Experience

Compensation and Benefits
