Home Health Physical Therapist - Full-Time in Princeton, NJ

Concentra PT and Wellness Clinic is actively looking for a dedicated and Licensed Physical Therapist (PT) to join our homecare team in Princeton, New Jersey. Work Environment

This position is predominantly onsite, providing therapy in the homes of our patients in Princeton and surrounding areas. The selected candidate will benefit from a supportive team environment, while also enjoying the autonomy that comes with home healthcare. The role allows for a flexible schedule from Monday to Friday, between 9 AM and 5 PM, accommodating both patients' and therapists' needs.

Key Responsibilities

• Conduct comprehensive in-home evaluations to assess patients' physical abilities and limitations.
• Develop tailored treatment plans that align with the specific rehabilitation goals of each patient, factoring in their unique circumstances.
• Implement therapeutic exercises, gait training, balance activities, and interventions aimed at enhancing functional mobility.
• Educate patients and their caregivers on essential strategies for home safety, fall prevention, and wellness, ensuring they understand their role in the recovery process.
• Continuously monitor patient progress, making adjustments to treatment plans based on functional goals and progress updates.
• Maintain accurate and detailed documentation of all patient visits in compliance with Medicare and organizational guidelines, ensuring smooth transitions of care.
• Collaborate effectively with a multidisciplinary team, including nursing staff, occupational therapists, speech-language pathologists, and social workers, to coordinate comprehensive patient care.

Patient Population / Client Focus

The Homecare Physical Therapist will work primarily with a diverse patient population, including individuals recovering from surgeries, those with chronic conditions, or patients needing rehabilitation for orthopedic, neurological, or other ailments. Qualifications

• Active New Jersey Physical Therapist License is required.
• A Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• A valid driver's license and access to a reliable vehicle with necessary auto insurance.
• Current CPR/BLS certification.
• Strong clinical judgment with excellent time-management capabilities.
• Proven ability to work independently in a home healthcare setting.

Preferred Experience

• A minimum of two years of experience in home health, outpatient, or hospital-based rehabilitation setting.
• Familiarity with electronic medical record (EMR) systems, such as HomeCare HomeBase, Kinnser, or MatrixCare.
• Knowledge of Medicare documentation and OASIS standards is preferred, but training will be provided.

Compensation and Benefits

Concentra offers competitive salary packages ranging from $90,000 to $110,000 annually, depending on experience. Additional benefits include:
