Work Environment

This position is based onsite in the communities we serve, allowing you to work directly in the home environment of our patients. Given the nature of home care, your day will involve traveling between patient homes, so flexibility and time management are vital. The expected work hours are Monday to Friday, from 9 am to 5 pm, providing a consistent schedule that encourages work-life balance.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions to develop tailored treatment plans aimed at restoring function and mobility.
• Provide one-on-one physical therapy sessions for patients, focusing on their unique recovery goals and needs.
• Utilize various therapeutic techniques, including but not limited to manual therapy, exercise programs, and assistive device training.
• Maintain an appropriate patient load to ensure quality care is delivered with ample time dedicated to each individual.
• Educate and motivate patients, families, and caregivers on self-management strategies and exercises to aid recovery.
• Implement evidence-based interventions, continually assessing patient progress and modifying care plans as necessary.
• Document all treatment interventions accurately using EMR software, particularly Web PT, to ensure thorough and compliant patient records.
• Collaborate with a multidisciplinary team of healthcare professionals to coordinate and enhance patient care.
• Advocate non-surgical solutions for patients, focusing on improving their quality of life and delaying the need for more invasive procedures.
• Encourage healthy lifestyle choices by promoting strength, flexibility, balance, and overall wellness.

Qualifications

• Graduation from an accredited Doctorate in Physical Therapy (DPT) program.
• An active and unencumbered state license for physical therapy practice.

• Proven ability to design, implement, and modify effective therapeutic exercise programs.
• Experience with EMR systems, particularly Web PT, to ensure accurate documentation and patient tracking.
• A solid understanding of current physical therapy methodologies backed by the latest research.

Preferred Experience

• Experience in home care or a similar outpatient setting is highly desirable.
• Demonstrated history of implementing successful treatment strategies in a variety of patient scenarios.
• Knowledge of additional specialties such as geriatric or pediatric therapy would be an advantage.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
