Key Responsibilities

• Direct and manage patient treatment plans that aim to prevent disability, relieve pain, and enhance muscle function based on individual assessments.
• Oversee the implementation of treatment protocols by physical therapy assistants to ensure compliance with established care objectives.
• Conduct comprehensive evaluations of patients' functional abilities through various diagnostic tests and assessments, formulating targeted rehabilitation strategies.
• Analyze the home environment for rehabilitation potential and provide actionable recommendations to promote patient independence.
• Deliver in-home physical therapy services as prescribed, including the administration of therapeutic exercises, manual therapy, traction, and other treatments.
• Utilize daily clinical documentation practices, including the preparation of evaluations, care plans, progress notes, and discharge summaries, according to agency protocols.
• Collaborate within an interdisciplinary team through case conferences, ensuring that the treatment plan aligns with the patient's needs and goals.
• Advise patients and their families on the use and maintenance of therapeutic equipment as well as their overall rehabilitation programs.
• Complete regular assessments and revisions of therapy plans based on patient progress, utilizing feedback from the entire care team.
• Be accessible to provide guidance to Physical Therapy Assistants during treatment sessions as mandated by state regulations.
• Participate in continuous quality improvement initiatives to enhance patient care and operational efficiency.

Patient Population / Client Focus

The Physical Therapist will primarily focus on an adult and geriatric patient population but may also interact with younger patients. These patients often present with a range of musculoskeletal conditions and require tailored therapy to recover from surgery, manage chronic pain, or improve their functional independence. Our providers take a comprehensive, patient-centered approach that values family input and aims to establish trust and rapport to aid in the rehabilitation process.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Licensed as a Physical Therapist in the state of New Jersey, with a commitment to maintaining an active license in good standing.
• Current certification in CPR and a dedication to upholding patient safety standards.
• A compassionate and collaborative approach to patient care, with excellent communication skills.
• Demonstrated ability to work effectively as part of a team focused on patient care and treatment objectives.

Preferred Experience

• A minimum of one (1) year of experience in physical therapy practice, preferably in an acute care or home care setting.

Compensation and Benefits

The position offers a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience.
