Work Environment

This full-time position is set in an onsite home care environment where you will directly impact patients' lives in the comfort of their homes. You will work Monday to Friday, collaborating within a multidisciplinary team to facilitate a smooth treatment process that aligns with each client's rehabilitation goals. Your role will involve not only providing therapy but also evaluating and adapting treatment plans to suit patients' evolving needs.

Key Responsibilities

• Direct the delivery of physical therapy treatment to prevent disabilities, relieve pain, and restore muscle function by implementing individualized care plans.
• Supervise and mentor physical therapy assistants to ensure adherence to prescribed therapy methods and safety regulations.
• Conduct comprehensive evaluations using diagnostic tests to assess muscle, nerve, and joint functions, developing tailored treatment strategies based on findings.
• Assess clients' home environments, providing recommendations that promote independence and safety during recovery.
• Deliver therapeutic exercises, including manual therapy techniques, to help clients achieve optimal functionality.
• Monitor and document changes in client conditions, collaborating with physicians and case managers to adjust treatment plans accordingly.
• Participate actively in interdisciplinary team meetings, contributing to holistic treatment planning that involves various healthcare professionals.
• Prepare accurate clinical documentation, including evaluations, care plans, and progress notes in compliance with health regulations.
• Empower clients and their families by providing education on the use and care of therapeutic devices, enhancing their engagement in the recovery process.
• Complete administrative tasks related to clinical documentation and patient records in a timely manner inline with branch policies.

Patient Population / Client Focus

You will serve a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or requiring rehabilitation due to age-related issues. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active and unencumbered Physical Therapy license in New Jersey.
• A minimum of one (1) year of experience as a physical therapist, preferably in acute care or home care settings.
• Current CPR certification and a commitment to maintaining professional competency through continuous learning.
• Strong decision-making skills, with the ability to lead and advocate for patient care.
• Excellent communication skills, capable of building rapport with clients and interdisciplinary team members.

Preferred Experience
