Work Environment

This position requires travel to patients' homes within Princeton and surrounding areas. You will adapt to a variety of home settings and be responsible for conducting treatments in a one-on-one environment. Our full-time schedule is Monday through Friday, from 9 AM to 5 PM, allowing for a structured yet flexible work routine that promotes work-life balance.

Key Responsibilities

• Conduct thorough in-home assessments to evaluate patients' physical conditions and capabilities, identifying functional deficits and setting realistic, achievable goals.
• Develop and implement individualized treatment plans that may include therapeutic exercises, gait training, balance retraining, and manual therapy techniques tailored to patients' unique needs.
• Instruct patients and caregivers on home exercise programs and educate them about their conditions, treatment processes, and preventive care strategies.
• Collaborate effectively with interdisciplinary teams, including physicians, nurses, and occupational therapists, to integrate care approaches and achieve optimal patient outcomes.
• Document patient progress, treatment modifications, and compliance accurately and promptly based on Medicare and agency standards to ensure continuity of care and regulatory compliance.
• Travel regularly to various patient residences, ensuring timely visits and a high quality of care throughout an assigned territory.

Qualifications

• Must hold a valid Physical Therapist license in New Jersey.
• A graduate of an accredited Physical Therapy program (DPT, MPT, or equivalent).
• Minimum of two years of physical therapy experience, preferably with a focus in home health settings.
• Possess a valid driver's license and reliable transportation for travel between patient locations.
• Exceptional communication, time management, and interpersonal skills essential for working independently as well as collaboratively.

Preferred Experience

• Experience in geriatric care, neurological rehabilitation, or orthopedic recovery.
• Previous exposure to home health documentation and compliance with Medicare and insurance requirements.
• Familiarity with electronic health record (EHR) systems to ensure accurate tracking of patient information and progress.

Compensation and Benefits

• Competitive salary ranging from $110,000 to $125,000 per annum, complemented by mileage reimbursement for travel.
