Work Environment

This position operates in a home care setting, allowing therapists to provide personalized treatment in the comfort and familiarity of each patient's home. The role will require travel between assigned patients' residences within the Princeton area. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to determine the appropriate treatment plans based on individual needs.
• Design, implement, and adapt treatment programs that address a variety of issues, such as orthopedic rehabilitation, pediatric conditions, neurologic disorders, and sports-related injuries.
• Provide detailed patient assessments and record keeping while ensuring compliance with all legal and ethical guidelines.
• Utilize state-of-the-art treatment techniques and modalities to facilitate patients' recovery and improve their functional abilities.
• Collaborate closely with other healthcare professionals, caregivers, and specialists to optimize patient outcomes through a multidisciplinary approach.
• Educate patients and their families about treatment plans, progress, and preventive care strategies to ensure understanding and adherence.
• Maintain patient confidentiality and uphold professional standards throughout all interactions.
• Meet or exceed utilization goals and actively participate in quality improvement initiatives to enhance service delivery.

Patient Population / Client Focus

The Physical Therapist will work with a diverse patient population, including individuals recovering from surgeries, injuries, or chronic conditions that impede mobility. You will provide therapeutic care tailored to each patient's unique circumstances while fostering trust and rapport that significantly enhances their recovery experience. Your ability to engage with patients on a personal level will empower them to actively participate in their healing process, ultimately driving better health outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.
• A current state license to practice as a Doctor of Physical Therapy (DPT).
• A minimum of two years' experience in a clinical setting, preferably with a focus on home care.
• Certification in CPR and BLS.
• Strong verbal and written communication skills to convey complex medical information clearly and effectively.
• Proficiency in using electronic health records and physical therapy documentation software.
• A genuine interest in building relationships with patients and their families to facilitate better health outcomes.

Preferred Experience

• Experience in diverse treatment modalities and a deep understanding of current physical therapy methodologies.
• Prior experience in home health care and familiarity with the challenges and rewards of delivering care in a patient's home.
• Ability to conduct community outreach and engage in health promotion efforts to enhance service visibility and access.

Compensation and Benefits
