Key Responsibilities

• Conduct thorough evaluations of patients, leveraging pertinent history and performing specialized assessments to inform treatment plans.
• Clearly communicate evaluation findings and treatment strategies to patients and their families, encouraging their participation in goal-setting processes to enhance engagement.
• Exhibit proficiency in executing treatment protocols based on evaluation findings, adapting based on real-time patient presentations and the evolving healthcare landscape.
• Collaborate with multi-disciplinary healthcare teams to optimize patient care, documenting any necessary modifications in treatment plans based on observed changes in patients' conditions.
• Oversee discharge planning, ensuring a seamless transition out of care alongside meticulous documentation of patient encounters, including evaluations, progress notes, and discharge summaries.
• Manage time effectively, meet established productivity benchmarks, and participate in team meetings, contributing insights and feedback as necessary.
• Support departmental goals through active participation in professional development initiatives and compliance with healthcare regulations.
• Serve as a role model and clinical instructor for students, guiding their practical experiences while continuing to pursue your professional growth.
• Promote a positive patient experience by being adaptable and responsive to individual needs, actively participating in initiatives to improve access to care.

Patient Population / Client Focus

This position primarily focuses on servicing a diverse population of patients in need of physical rehabilitation following injuries, surgeries, or chronic health conditions. Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid Physical Therapy license in the State of New Jersey is essential.

Preferred Experience
