Key Responsibilities

As a Physical Therapist, your daily responsibilities will contribute significantly to the health and functionality of your patients. Your key tasks will include:

• Conducting in-depth assessments of patients' physical abilities and limitations to develop personalized care plans in collaboration with the health care team.
• Implementing individualized therapy interventions, ensuring each session addresses specific rehabilitation goals established in coordination with referring physicians.
• Employing skilled therapeutic techniques and exercises to improve patients' mobility and quality of life in the comfort of their homes.
• Educating patients and their families or caregivers about therapeutic exercises, necessary equipment, and home safety measures to promote ongoing recovery.
• Prioritizing your daily caseload, coordinating staffing needs, and ensuring smooth transitions of care through effective hand-off communication.

Patient Population / Client Focus

You will be working with a diverse population, including pediatric patients recovering from surgery and older adults managing chronic conditions. Qualifications

To be successful in this role, you must possess the following qualifications:

• A valid state license in Physical Therapy.
• A Doctor of Physical Therapy (DPT) degree.
• Certification in Basic Cardiac Life Support (BCLS) is required within 30 days of hire.
• Although previous experience is not required, applicants should be passionate about delivering exceptional patient care.

Preferred Experience
