Work Environment

This position offers an onsite work setting primarily focused on home care services, allowing therapists to deliver personalized treatment to patients in the comfort of their own homes. Key Responsibilities

As a Physical Therapist at Aviant, you will manage a wide range of responsibilities:

• Conduct comprehensive evaluations, assessing patients' medical history, performing clinical assessments, and documenting findings to establish a tailored treatment plan.
• Clearly communicate evaluation outcomes and treatment strategies to patients and their families, including setting realistic and achievable goals together.
• Implement and adapt treatment protocols effectively based on each patient's progress and needs, utilizing evidence-based practices to ensure optimal patient outcomes.
• Collaborate with an interdisciplinary team by consulting with other healthcare providers when necessary and adjusting care plans as required based on ongoing assessments.
• Maintain accurate and timely documentation of all patient care activities, ensuring a clear and organized record of the patient's course of treatment.
• Exhibit strong time management skills to meet productivity standards while maintaining a commitment to compassionate patient care.
• Proactively engage in initiatives to enhance patient accessibility and quality of care by removing barriers and utilizing site-level data effectively.
• Supervise and mentor students during clinical placements, contributing to their professional development and growth through hands-on guidance.
• Participate in community outreach and marketing efforts to raise awareness of our services and promote wellness in the local area.

Patient Population / Client Focus

Your primary focus will be on diverse patient populations experiencing a range of conditions from post-operative recovery to chronic pain management. You will work with adult and elderly clients, requiring a personalized approach in treatment to improve their mobility, function, and quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; candidates with a Doctorate in Physical Therapy (DPT) are preferred.
• A valid Physical Therapist license in New Jersey is required to practice.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Commitment to continued professional education and personal growth.

Preferred Experience

While a minimum of two years of previous experience as a licensed Physical Therapist is preferred, we welcome applicants who are enthusiastic about the role and eager to make a positive impact in patients' lives, regardless of prior experience.

Compensation and Benefits
