We are seeking a dedicated and skilled Orthopedic Physical Therapist to join our team at Ascove Injury Prevention Rehab Center in Trenton, New Jersey. This full-time role offers an exciting opportunity for you to provide exceptional patient care through home-based therapies, with a competitive salary range of USD 100,000 to 120,000 annually. Work Environment

This position is primarily onsite and involves delivering care directly in patients' homes. The work schedule consists of 40 hours per week with variable weekends as required to meet patient needs. Key Responsibilities

• Conduct thorough assessments of patients to evaluate their functional limits and develop individualized treatment plans tailored to their needs.
• Provide ongoing consultations to various healthcare providers regarding patient progress and continuum of care initiatives.
• Adhere to customer service standards by interacting respectfully and effectively with patients of all ages and backgrounds.
• Implement treatment plans while continuously monitoring patient responses and adjusting as necessary; maintain accurate documentation of evaluations, treatments, and patient interactions.
• Collaborate with an interdisciplinary team—including physicians and support staff—to discuss patient care strategies and share important updates.
• Lead educational sessions for staff, patients, and community members to promote awareness and understanding of orthopedic rehabilitation.
• Contribute to program development, quality improvement initiatives, and community outreach programs.
• Attend regular staff meetings and participate actively in maintaining and enhancing your professional competencies through continuing education and self-study.

Patient Population / Client Focus

Your primary focus will be on patients with orthopedic conditions requiring rehabilitative services. This includes individuals recovering from surgeries, injuries, and chronic pain conditions. You will tailor your therapeutic techniques to ensure optimal recovery outcomes while addressing each patient's unique needs and preferences.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate in Physical Therapy is preferred for advanced patient interaction and evaluations.
• Valid licensure in the state of New Jersey and clean standing with the regulatory board.
• Strong written and verbal communication skills are essential for clear documentation and effective patient interactions.
• Proficiency in electronic medical records (EMR) systems for maintaining accurate patient information.

Preferred Experience

• Prior experience in orthopedic physical therapy, especially within a home care setting, is highly beneficial.
• Knowledge of current orthopedic rehabilitation methodologies and evidence-based practices.
• Experience managing patient care as part of a multidisciplinary team is advantageous.

Compensation and Benefits
