Work Environment

This position is situated within the home care setting, allowing for personalized therapy sessions. As a Physical Therapist at Liberty Therapy Clinic, you will work in partnership with patients in the comfort of their homes, which enhances the therapeutic relationship and allows for tailored treatment plans. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to assess their physical capabilities and rehabilitation needs.
• Develop and implement individualized treatment programs that focus on improving mobility, alleviating pain, and restoring function according to clinical guidelines and physician directives.
• Engage with a multidisciplinary team to ensure optimal patient outcomes, collaborating with other healthcare professionals when necessary.
• Utilize various therapeutic techniques and modalities to facilitate recovery, which may include manual therapy, exercise programming, and patient education.
• Educate patients and their caregivers about treatment plans, fostering a collaborative approach to achieving rehabilitation goals.
• Ensure adherence to quality care metrics and practice standards established by Liberty Therapy Clinic and relevant regulatory bodies.
• Achieve and exceed established utilization goals and contribute to overall clinic success through service excellence.

Patient Population / Client Focus

The Orthopedic Physical Therapist will be focused on treating a diverse patient population, which includes individuals with orthopedic injuries, chronic pain conditions, sports-related injuries, and post-surgical rehabilitation needs. You will have the flexibility to work with different age groups, from pediatric to geriatric patients, allowing for a rewarding variety in day-to-day interactions and treatments.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program is required.
• Current licensure as a Doctor of Physical Therapy (DPT) by the state of New Jersey, in accordance with the Physical Therapy Examining Committee.
• A minimum of two (2) years of experience in a physical therapy role, with a strong emphasis on orthopedic rehabilitation.
• Current CPR certification to ensure patient safety and emergency preparedness.

Preferred Experience

While a minimum of two years' experience is required, candidates with experience in home health care settings will find this role especially fitting. Familiarity with electronic health record (EHR) systems and patient management software will also be advantageous.
