Key Responsibilities

• Conduct thorough patient evaluations by reviewing medical history, performing specialized assessments, and establishing a personalized treatment plan based on documented findings.
• Effectively communicate evaluation results and treatment plans to both patients and their families, engaging them actively in the goal-setting process.
• Implement evidence-based treatment protocols, adapting them as necessary to meet the evolving needs of each patient within a dynamic healthcare landscape.
• Consult with multidisciplinary teams and document significant changes in patients' conditions, ensuring timely modifications to care plans and referrals for additional medical or rehabilitation services.
• Manage discharge planning and follow-up care, ensuring that documentation reflects a clear narrative of the patient's treatment course, including evaluations, progress updates, and discharge summaries.
• Adhere to time management principles, meeting productivity standards while maintaining a well-structured schedule that facilitates seamless workflow and patient access.
• Contribute to achieving departmental goals and compliance with all regulatory requirements, actively participating in quality improvement initiatives.
• Engage in ongoing education and skill enhancement, taking the lead in mentoring students and new hires as a clinical instructor.
• Promote an exceptional patient experience by being flexible and responsive to individual care needs, participating actively in efforts to improve access to care.

Patient Population / Client Focus

The Physical Therapist will work with a diverse patient population, including individuals recovering from surgery, sports injuries, neurological conditions, and chronic pain disorders. Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.

Preferred Experience

Compensation and Benefits
