Work Environment

This position is primarily an onsite role focused on home care, meaning you will travel to various patient residences in Charlottesville. Our supportive clinic environment ensures that our Physical Therapists have direct access to resources and a network of healthcare professionals, facilitating interprofessional collaboration and comprehensive patient care.

Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing their medical history, performing assessments, and documenting findings to create a tailored treatment plan.
• Clearly communicate evaluation results and the proposed treatment plan to both patients and family members, actively involving them in goal setting to promote engagement and compliance.
• Implement treatment protocols with proficiency based on individual evaluations and the latest practices in physical therapy, ensuring flexibility to adapt to changing patient needs.
• Consult with other healthcare professionals when necessary, monitoring patients' conditions and documenting any changes that may require modifications to their care plans.
• Manage discharge planning efficiently, ensuring that patients and their families are supported through the transition process, while also maintaining accurate and timely documentation.
• Exhibit effective time management skills, meeting productivity expectations while adhering to appointment schedules.
• Engage in community outreach by participating in marketing activities, promoting the clinic's services, and contributing to the development of departmental goals.
• Pursue ongoing professional development by participating in continuing education opportunities and serving as a mentor to clinical students.
• Champion a superior customer experience by demonstrating adaptability in addressing patient needs and removing barriers to access.

Patient Population / Client Focus

This role will focus on a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or experiencing age-related physical challenges. Your approach will prioritize compassionate care, enabling each patient to regain independence and functional mobility in their daily lives.

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
• Must hold a valid Physical Therapy license in the state of Virginia.
• Current American Heart Association's BLS for Healthcare Providers certification is mandatory.
• Demonstrated commitment to continued education and professional growth.

Preferred Experience

• A minimum of two years of physical therapy experience, particularly in a home care or outpatient setting, is preferred but not mandatory.
• Experience working collaboratively within a multidisciplinary team to address diverse patient needs effectively.

Compensation and Benefits

The salary for this position ranges from $100,000 to $120,000 annually, commensurate with experience.
