Work Environment

This position is onsite, providing home care services in the beautiful Charlottesville area. You will interact closely with patients in their homes, enabling a unique approach that fosters comfort and convenience for patient rehabilitation. Working Monday through Friday, you will enjoy a balanced schedule that enhances work-life integration. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, which include reviewing health histories and performing specific assessments to determine individualized treatment recommendations.
• Communicate evaluation findings transparently with patients and their families, ensuring they are integral to the goal-setting process for recovery.
• Implement effective treatment protocols based on evaluation findings and adapt these as necessary to align with ongoing changes in the patient's health status.
• Collaborate with other healthcare professionals, consulting on treatment modifications when patient conditions change, and documenting all developments accurately.
• Develop detailed discharge plans that ensure continuity of care, including necessary follow-up appointments and rehabilitation strategies.
• Maintain meticulous documentation of treatment progress, including clear initial evaluations, daily notes, and comprehensive discharge summaries that adhere to regulatory standards.
• Efficiently manage your schedule, meeting productivity expectations while remaining punctual for all engagements, and participating in marketing initiatives within the community.
• Contribute to departmental goals and compliance with healthcare regulations, while actively pursuing continuing education opportunities to enhance your professional development.
• Deliver exceptional customer service, demonstrating flexibility and adaptability to ensure optimal patient access to care.
• Engage in initiatives that address community health accessibility, actively participating in programs designed to remove barriers to healthcare.

Patient Population / Client Focus

You will work with a diverse patient population, including those recovering from surgeries, injuries, or chronic conditions that significantly impact their mobility and daily activities. Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current license to practice as a Physical Therapist in Virginia, ensuring compliance with all state regulations.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

While a bachelor's degree is required, candidates with additional certifications or specializations in physical therapy modalities, prior experience in home care settings, or familiarity with community health initiatives will be given preference.
