Work Environment

This Full-Time position operates primarily in a home care setting, providing you with the opportunity for one-on-one patient interactions in a comfortable environment. Our onsite approach allows therapists to closely monitor and adjust treatment plans based on patients' immediate responses to therapy. The job requires travel to patients' homes within Charlottesville, enhancing the patient-therapist relationship and allowing for a more tailored approach to rehabilitation.

Key Responsibilities

• Conduct comprehensive evaluations of patients, taking into account their medical history, current physical functions, and personal goals, to create individualized treatment plans.

• Develop and implement therapeutic interventions consistent with physician orders, actively adjusting methods and goals to meet patients' evolving needs.

• Maintain accurate documentation of patient care activities, including evaluation results, treatment progress, and discharge summaries, using electronic health record (EHR) systems to ensure compliance and continuity of care.

• Educate patients and their families on treatment goals, techniques, and the importance of adherence to prescribed exercises and lifestyle changes for optimal outcomes.

• Collaborate with a multidisciplinary team including physicians, nurses, and occupational therapists, to provide a holistic approach to patient care and address complex needs.

• Supervise Physical Therapist Assistants and support personnel, providing guidance and support to optimize patient care and treatment outcomes.

Patient Population / Client Focus

The clients you will serve primarily include individuals recovering from surgeries, injuries or managing chronic illnesses, all requiring specialized physical therapy interventions to regain independence and improve their quality of life. Your expertise will be crucial in evaluating and addressing a diverse range of patient needs across different age groups, ensuring tailored and compassionate care.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.

• Current licensure as a Physical Therapist in the State of Virginia.

• Strong communication skills, both written and verbal, alongside proficiency in using technology for documentation and communication.

Preferred Experience

• A minimum of 2 years of experience in a clinical setting, particularly home health or community health environments, is preferred.

• Experience working within a multidisciplinary team and with electronic health record (EHR) systems.

Compensation and Benefits
