Work Environment

This position conducts therapy sessions onsite at patients' homes in Charlottesville, Virginia. Our approach relies on the ability to adapt therapy plans based on individual environments, ensuring that the care provided aligns with the patient's surroundings. You will work closely with patients, families, and interdisciplinary teams to foster a collaborative and effective therapeutic journey tailored to each client's specific needs.

Key Responsibilities

• Direct comprehensive physical therapy treatments aimed at preventing disabilities, relieving pain, and enhancing muscle function, while adhering strictly to individual patient care plans.
• Evaluate patient functionality using advanced diagnostic methods to assess muscle, nerve, and joint abilities, and devise targeted rehabilitation strategies accordingly.
• Conduct home assessments to determine the suitability of environments for rehabilitation, providing actionable recommendations tailored to promote independence.
• Administer therapy services in accordance with physician directives, employing hands-on techniques such as manual therapy, exercise programs, and functional activities.
• Collaborate with physical therapy assistants, guiding them in the execution of care plans and maintaining ongoing supervision compliant with state regulations.
• Maintain meticulous clinical records, including evaluations, progress notes, and discharge summaries, ensuring all documentation is timely and precise.
• Participate actively in interdisciplinary case conferences to evaluate treatment efficacy and coordinate patient care effectively.

Patient Population / Client Focus

Qualifications

• Current CPR certification is mandatory.
• A valid driver's license and personal automobile insurance are required for travel between patient homes.

Preferred Experience

• A minimum of one year of clinical experience as a Physical Therapist in home care or acute care settings is preferred.
• Experience in using electronic health record (EHR) systems for documenting patient care and treatment plans.
• Proven ability to communicate effectively and build rapport with patients and caregivers.
