Licensed Physical Therapist for Home Care Services

Work Environment

This role is based onsite, primarily within clients' homes, which fosters a personalized approach to rehabilitation. You will have the opportunity to assess the home environment for rehabilitation needs and provide therapy in a familiar setting for clients, ensuring comfort and increased engagement in their care process. Our Home Care setting allows you to use a variety of tools and methodologies tailored specifically to each client, contributing significantly to their recovery journey.

Key Responsibilities

Your responsibilities will include, but are not limited to:

• Directing physical therapy treatment plans aimed at preventing disabilities, relieving pain, and restoring muscle function as per established care guidelines.
• Evaluating clients' functional levels through comprehensive tests on muscles, joints, and overall physical capabilities.
• Overseeing and mentoring Physical Therapy Assistants in executing appropriate therapeutic strategies, ensuring adherence to the treatment plan.
• Conducting in-home functional assessments, making recommendations for modifications to the home environment that promote client independence.
• Providing a variety of therapeutic services, including manual exercises, traction, massage, and other physical agents to aid in the rehabilitation process.
• Documenting client evaluations, progress notes, care plans, and discharge summaries as part of ongoing clinical record maintenance according to regulatory standards.
• Engaging as a key member of the interdisciplinary healthcare team, participating in case conferences and care plan reviews to optimize client treatment.

Patient Population / Client Focus

You will work primarily with a diverse client population, including individuals recovering from surgery, managing chronic illnesses, or those seeking to enhance their mobility and independence post-injury. The compassionate and family-centered approach is essential, as you will collaborate closely with clients and their families to educate them about the therapy process and involve them actively in achieving their health goals.

Qualifications

To qualify for this role, you must possess:

• A Doctorate degree in Physical Therapy (DPT).
• A valid and current license to practice as a Physical Therapist in the state of Virginia.
• Current CPR certification to ensure patient safety during therapy sessions.
• A valid driver's license and automobile insurance for commuting to client locations.
• A strong commitment to clinical excellence and a positive attitude that fosters teamwork and collaboration.

Preferred Experience

We prefer candidates with:

• Two or more years of experience as a Physical Therapist in home care or acute care settings.
• Experience using outcome-based methodologies that measure and demonstrate client progress effectively.
• A demonstrated ability to engage with diverse patient populations, including geriatric clients and those with complex medical histories.
• Strong interpersonal skills that promote effective communication and relationship-building with clients, families, and healthcare teams.

Compensation and Benefits
