Work Environment

This position is full-time and requires on-site work, as you will be providing care directly within patients' homes, which allows you to truly understand their unique environments and challenges. The home care setting presents a variety of encounters, wherein you can tailor therapy approaches to each patient's specific needs, promoting healing and restoring function in a comfortable and familiar space.

Key Responsibilities

• Evaluate patients referred by physicians to develop customized physical therapy treatment plans based on thorough assessments.
• Implement and modify treatment programs, utilizing evidence-based practices to achieve optimal therapeutic outcomes.
• Administer therapy techniques with a focus on restoring function — including strength training, mobility exercises, and pain management strategies.
• Educate and train patients, alongside their families, on how to manage conditions and implement home exercises to promote recovery and independence.
• Demonstrate exceptional customer service and maintain high standards of patient care while working collaboratively with a broader healthcare team.
• Document patient progress meticulously and meet minimum utilization goals and other performance objectives as set by the organization and governing bodies.

Patient Population / Client Focus

Your role will encompass treating a diverse clientele, with patients spanning various age groups and rehabilitation needs, including orthopedic, neurologic, pediatric, and sports medicine cases. Qualifications

• Graduation from an APTA or CAPTE-accredited physical therapy program.
• Bachelor's Degree in Physical Therapy, with a Doctorate in Physical Therapy (DPT) preferred.
• Active State License in Physical Therapy and Basic Life Support Certification from the American Heart Association.
• Strong communication skills, both written and verbal, paired with proficient computer skills for documentation and record-keeping.
• Ability to build rapport and establish trust with each patient, ensuring they feel valued and supported.

Preferred Experience

• A minimum of 2 years of experience in physical therapy, preferably within a home care setting.
• Prior experience working with a multidisciplinary team focused on patient-centered care.
• Demonstrated skill in utilizing current therapeutic techniques and protocols to improve patient outcomes.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 annually, complemented by a comprehensive benefits package.
