Work Environment

The position is an onsite role focused on home care, which allows our physical therapists to deliver care directly in the homes of our patients, thereby enhancing their comfort and recovery experience. Your workweek will typically consist of 40 hours, with variable weekend shifts as needed. Key Responsibilities

Your day-to-day responsibilities will include:

• Conducting thorough examinations and evaluations of patients with orthopedic conditions, assessing their functional limitations and disabilities.
• Developing individualized treatment plans based on evaluation outcomes, outlining clear goals and interventions tailored to each patient.
• Delivering physical therapy treatments, educating patients, and caregivers about their conditions and how to manage them effectively at home.
• Monitoring patient progress by conducting regular re-evaluations and modifying treatment plans as required.
• Communicating effectively with interdisciplinary teams, including physicians and other healthcare providers, to ensure coordinated care.
• Documenting all patient interactions, treatment outcomes, and relevant administrative details in compliance with legal standards and organizational policies.
• Participating in community outreach programs, quality improvement initiatives, and professional development activities to enhance team performance.
• Overseeing and mentoring less experienced staff and students in their clinical education and treatment methodologies.
• Engaging in professional development activities, including attending workshops and certification programs to enhance clinical skills.

Patient Population / Client Focus

Our primary focus is on patients with orthopedic conditions, including post-surgical rehabilitation and chronic pain management. You will work closely with a diverse patient population, providing services to individuals of all ages, backgrounds, and abilities. Your role is vital in contributing to improved patient outcomes and quality of life through tailored physical therapy interventions.

Qualifications

To qualify for this position, candidates must possess:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid State License in Physical Therapy and current BLS certification.
• Strong written and verbal communication skills to effectively interact with patients and team members.
• Proficiency in word processing and electronic health record systems to maintain documentation accuracy.
• Experience in orthopedic physical therapy preferred.

Preferred Experience

We are looking for candidates with at least two years of experience in a clinical setting, ideally with a focus on orthopedic rehabilitation. Demonstrated ability to manage a diverse patient caseload and provide patient education are key attributes we value in our candidates.

Compensation and Benefits
