Work Environment

This position will primarily be based in the home care setting, where you will deliver therapy directly in patients' residences. Key Responsibilities

• Conduct comprehensive evaluations, gathering and analyzing relevant medical history, performing detailed assessments, and developing tailored treatment plans to address each patient's specific needs and goals.
• Communicate evaluation findings and treatment strategies effectively to patients and their families, actively involving them in goal-setting processes to encourage engagement and motivation.
• Implement treatment protocols efficiently, adjusting methods based on the patient's progress and changes in their condition, utilizing evidence-based practices to ensure optimal outcomes.
• Maintain close collaboration with other healthcare professionals, documenting changes in patients' conditions and making necessary adjustments to care plans, including referrals for additional medical or rehabilitative consultations.
• Ensure thorough discharge planning, including follow-up care, and document all patient interactions accurately and timely, contributing to a cohesive understanding of patient care history and progress.
• Manage time effectively, adhering to productivity standards and maintaining a punctual schedule while participating in team meetings and marketing activities within the community.
• Contribute to departmental goals through active participation in quality improvement initiatives and compliance with accreditation standards and governmental regulations.
• Engage in professional development by seeking continuing education opportunities and mentoring students or less experienced staff.
• Foster an exceptional customer experience, addressing barriers to access and employing strategies to improve patient satisfaction as indicated by outcome measures.

Patient Population / Client Focus

The primary focus of this role is to serve a diverse patient population in the home health setting, including elderly individuals, those recovering from surgery, and patients with chronic conditions requiring ongoing rehabilitation. The physical therapist will work closely with patients recovering from stroke, orthopedic surgeries, and various musculoskeletal issues, emphasizing functional mobility and independence in daily activities.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapy license in the state of Virginia, with an understanding of the regulations governing practice in home care settings.
• Current CPR certification through the American Heart Association's BLS for Healthcare Providers.
• A strong foundation in therapeutic modalities and treatment techniques directly applicable to home health therapy.

Preferred Experience

• A minimum of 2 years working as a physical therapist in a home care, rehabilitation, or acute care setting preferred.
• Experience in developing individualized treatment plans and familiarity with electronic health record (EHR) systems for timely documentation.

Compensation and Benefits
