Work Environment

This position is primarily a home care role, meaning you will interact with patients in their own environments where they feel most comfortable and safe. Key Responsibilities

• Conduct thorough evaluations of patients recovering from injuries, surgeries, or chronic conditions in their homes to determine their specific rehabilitation needs.
• Develop and implement individualized treatment plans using a variety of therapeutic modalities, including exercise programs, stretching techniques, hands-on manual therapy, and specialized equipment designed to alleviate pain and enhance mobility.
• Monitor and document patient progress rigorously, adjusting treatment plans based on evaluations and feedback to ensure optimal outcomes.
• Provide education and support to patients, caregivers, and family members about treatment goals and home exercises to encourage active participation in the rehabilitation process.
• Advise on the use of assistive devices, ensuring that patients are fit properly for any equipment necessary to aid their recovery.
• Stay informed of best practices and advancements in physical therapy through continuous professional development, including attending workshops and engaging in networking opportunities.

Patient Population / Client Focus

Our clientele includes a diverse range of individuals, from young children to seniors, who present with various orthopedic concerns such as post-surgical recovery, sports injuries, and chronic pain conditions. You will play a crucial role in helping these patients regain their independence and improve their functionality within their home and community, tailoring interventions based on their unique lifestyles and goals.

Qualifications

• A valid Doctorate in Physical Therapy (DPT) is required.
• Active state licensure in Physical Therapy.
• Strong communication and organizational skills, both written and verbal.
• Proficiency in word processing and basic computer skills to maintain accurate patient records.
• Flexibility and adaptability in a fast-paced environment to meet the varying needs of patients.

Preferred Experience

Compensation and Benefits
