Joint Ventures Physical Therapy is seeking a dedicated and skilled Home-Based Orthopedic Physical Therapist to join our exceptional team in Freehold, New Jersey. The ideal candidate will possess a Bachelor's degree in Physical Therapy and at least one year of clinical experience in physical therapy, preferably within home care settings.

Work Environment

This position operates onsite within the home environment of our clients, providing a unique opportunity to build meaningful relationships while helping individuals achieve their rehabilitation goals. Working Monday through Friday, the flexibility of the role supports a balanced work-life integration, allowing therapists to engage with clients in a personalized and comfortable setting. Key Responsibilities

• Conduct comprehensive evaluations of each client's physical status to develop tailored treatment plans focused on preventing disability, relieving pain, and restoring muscle function.
• Direct and oversee physical therapy assistants, ensuring adherence to treatment plans and high standards of care.
• Utilize diagnostic tests to evaluate muscle, nerve, joint, and functional abilities, enabling targeted treatments that enhance mobility.
• Assess home environments for rehabilitation effectiveness and make recommendations to promote client independence.
• Administer effective therapeutic exercises, both manual and non-manual, guiding and motivating clients through their rehabilitation exercises and functional activities.
• Apply physical agents such as traction and massage to facilitate recovery, while monitoring client responses to therapy.
• Evaluate, fit, and adjust prosthetic and orthotic devices as necessary, collaborating with physicians to ensure optimal client outcomes.
• Create, maintain, and update clinical records including evaluations, care plans, and progress notes, adhering to all documentation standards and policies.
• Engage with an interdisciplinary team during case conferences to discuss treatment plans, challenges, and strategies for client success.
• Consult with clients and their families to educate them about therapy devices, programs, and their overall rehabilitation journey.
• Complete functional assessments and revisions of care plans, ensuring utilization of community resources when necessary.
• Adhere to all regulatory and organization policies, submitting required clinical and payroll documentation promptly.
• Supervise physical therapy assistants in specific treatment phases, maintaining compliance with state law requirements.
• Fulfill additional responsibilities as required to achieve organizational objectives and improve client care.

Patient Population / Client Focus

Qualifications

• Graduate from an accredited physical therapy program with a Bachelor's degree in Physical Therapy.
• One year of clinical experience as a physical therapist within an acute care or home care setting.
• Current Physical Therapy licensure in New Jersey (or state of practice) in good standing.
• Current CPR certification to ensure readiness in emergency situations.
• A valid driver's license and automobile insurance for travel between client homes.
• Creativity in problem-solving and a proactive approach in patient care.
• Capable decision-maker willing to take responsibility for patient outcomes.

Preferred Experience

Previous experience in a home care environment is highly valued, particularly in managing orthopedic conditions. Familiarity with electronic health record systems and a strong commitment to clinical excellence will enhance success in this role.

Compensation and Benefits

Joint Ventures Physical Therapy offers a competitive salary range of USD 100,000–120,000 annually, commensurate with experience.
