Key Responsibilities\n

• Conduct comprehensive assessments to identify the specific needs of your patients, customizing treatment approaches accordingly.

• Collaborate with supervisors, direct care staff, and families to communicate individual progress effectively, fostering a supportive network for clients.

• Establish a strategic schedule for appointments and home visits, ensuring consistent evaluations and monitoring of client advancements.

• Oversee home visits to conduct in-depth observations and interviews, as well as facilitate chart reviews to ensure comprehensive data analysis.

• Implement program plans and regularly evaluate their effectiveness, ensuring that clients receive the highest level of care.

• Participate in interdisciplinary team meetings to contribute insights and strategies that shape patient-focused care.

• Complete detailed monthly and quarterly progress notes that track client development and therapeutic outcomes.

• Organize training sessions to ensure that staff members are equipped with the skills necessary for implementing treatment plans and documentation processes effectively.

• Maintain thorough records to guarantee accessibility and readiness of information for stakeholders involved in client care.

• Act as a liaison between clients, families, and the greater mental health community to support broader developmental goals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) within the state of Georgia.
