Key Responsibilities\n

As a Mental Health Counselor at Medic Care & Health Services, your key responsibilities will include:

• Conduct thorough assessments to identify and address the individual needs of your clients, ensuring a personalized approach to therapy.

• Engage in effective communication with supervisors, care providers, and family members to discuss clients' progress and collaborate on treatment plans.

• Develop and manage a tentative schedule for appointments, home visits, and observations to monitor and support client progress.

• Coordinates and conducts home visits to execute in-home assessments and gather pertinent information through observations and chart reviews.

• Ensure that the therapeutic program plans are implemented effectively and client goals are achieved.

• Participate in interdisciplinary team meetings to contribute to discussions surrounding client outcomes and program enhancements.

• Document client progress through monthly and quarterly reports, maintaining accurate records for compliance and review.
\n
• Lead training sessions for staff to ensure competence in program plan implementation and documentation processes.

• Provide ongoing support through follow-up interviews and additional home visits to reinforce therapy objectives.

• Act as a liaison to the mental health community, promoting service development and resource sharing.

Qualifications\n
