Key Responsibilities\n

• Conduct thorough assessments to evaluate the counseling and therapeutic needs of clients, employing best practices and diagnostic criteria.

• Utilize DSM (Diagnostic and Statistical Manual) criteria to accurately diagnose substance use issues and determine appropriate levels of care in accordance with ASAM (American Society of Addiction Medicine) standards.

• Develop and implement tailored treatment plans focusing on substance abuse counseling that aligns with the client’s unique needs and recovery goals.
\n
• Facilitate discharge and continuum of care planning to ensure clients successfully transition to the appropriate level of support after treatment.
\n
• Maintain meticulous documentation of all counseling services, including treatment plans, updates, referrals, and outcomes in each client’s clinical record.
\n
• Manage a designated caseload effectively, ensuring adherence to treatment planning and clinical charting requirements.
\n
• Coordinate support services and communicate effectively with clients’ families and external resources, documenting interventions provided.
\n
• Engage with referral sources, including legal and educational institutions, to provide a holistic support network for clients.
\n
• Implement crisis intervention strategies using a multidisciplinary team approach to address immediate client needs.

Patient Population / Client Focus\n

The primary focus of this role is on individuals suffering from co-occurring mental health disorders and substance use disorders. As an LCSW, you will work closely with clients who may be experiencing various challenges related to addiction, trauma, and emotional distress.
