Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, families, and groups, identifying client needs and developing tailored treatment plans.

• Perform initial assessments, setting the stage for effective triage and coordinated care, using evidence-based practices.

• Document and communicate assessment outcomes and treatment interventions with colleagues and interdisciplinary teams, participating in regular patient rounds and meetings to ensure cohesive patient care.

• Utilize your clinical knowledge to address the psychological, social, and educational needs of various aged clients, ensuring ethical and competent service delivery.

• Manage a caseload of patients by coordinating with community resources and healthcare professionals to offer holistic support and comprehensive care pathways.

• Maintain timely and accurate documentation in compliance with department standards while ensuring confidentiality and ethical guidelines are upheld throughout all interactions.

• Employ Cognitive Behavioral Therapy (CBT) and other evidence-based modalities to assist clients in achieving their individualized service goals.

Patient Population / Client Focus\n

You will work with a diverse array of clients, ranging from children and adolescents to adults, each requiring customized therapeutic approaches. Our outpatient setting allows for the treatment of various mental health challenges, encompassing anxiety, depression, trauma, and relational difficulties. Your role directly contributes to improving individual wellbeing and enhancing overall mental health within the Sacramento community.

Qualifications\n

• A valid license as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in the State of California.
