Key Responsibilities\n

• Coordinate with Reentry Social Workers to develop effective discharge planning services for patients.
\n
• Prepare and complete required statistical reports that inform treatment effectiveness and patient care strategies.
\n
• Collaborate with the Department of Correction to facilitate access to patients, ensuring they receive appropriate housing and treatment options.
\n
• Integrate patient care with other institutional programs, developing and leading group therapy sessions focused on mental health observations.
\n
• Maintain accurate documentation in compliance with regulatory standards and participate in weekly clinical supervision sessions with the Clinical Supervisor.
\n
• Engage in mental health and medical rounds to stay updated on patient statuses and treatment plans.
\n
• Contribute to case conferences and treatment planning discussions for patients currently receiving or being considered for mental health treatment.
\n
• Administer psychological testing as needed and provide supervision for social work interns or student volunteers when applicable.
\n
• Conduct thorough mental health evaluations, crisis interventions, and ongoing assessments, ensuring continuity of care through effective follow-up.

• Evaluate and determine the proper disposition of patients, facilitating their transition to the appropriate level of mental health care based on assessed needs.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at NewBetterYou, you will primarily support patients within a hospice setting, including children and teenagers.
