This remote position allows you to work flexibly from your home, while providing care to clients at a Hospice Center when necessary. Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify clients' mental health challenges, strengths, and available resources.

• Facilitate individual, family, and group therapy sessions, utilizing evidence-based treatment modalities tailored to each client’s specific needs.
\n
• Develop and implement individualized treatment plans that outline therapeutic goals and actionable interventions, guiding clients through their recovery process.

• Collaborate closely with psychiatrists, psychologists, and multidisciplinary treatment teams to ensure comprehensive and coordinated care for every client.
\n
• Provide crisis intervention, perform risk assessments, and create safety plans for clients in acute distress.
\n
• Maintain meticulous and timely documentation in accordance with ethical, legal, and organizational standards, ensuring quality and compliance.

• Lead psychoeducation classes and support groups, empowering clients with knowledge and skills to manage their mental health effectively.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including individuals experiencing various mental health disorders, ranging from anxiety and depression to complex trauma and grief related to hospice care. You will play a pivotal role in supporting clients of all ages, fostering resilience and encouraging healing through tailored therapeutic interventions.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

• At least two years of relevant experience in a mental health or outpatient setting is required.

• Strong familiarity with evidence-based therapeutic modalities and best practices in mental health treatment.

Compensation and Benefits\n
