Key Responsibilities\n

• Psychosocial Assessment: Conduct timely and comprehensive assessments by utilizing theoretical knowledge of human behavior, psychosocial factors, and ethical considerations impacting treatment plans.

• Clinical Intervention: Provide supportive counseling to patients and families, facilitating difficult conversations about care decisions, including hospice, DNR orders, and placement. Collaborate with Case Managers on discharge planning and develop group programs to enhance community resources.

• Caseload Management: Effectively identify and manage high-risk situations, ensuring balanced attention to referrals and ongoing patient care while seeking assistance from colleagues as necessary.

• Legal and Ethical Standards: Ensure adherence to legal and ethical protocols in all clinical assessments, including mandated reporting and patient rights, while complying with organizational policies and practices in social services.

• Documentation: Maintain accurate and timely documentation in accordance with policy requirements, including interdisciplinary care plans and social services progress notes.

• Education and Training: Deliver in-service training to hospital staff about psychosocial issues affecting patient care, contributing to continuous education within the organization.

• Quality Improvement: Participate in the development and analysis of performance indicators aimed at enhancing patient care quality and organizational effectiveness.

Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from $120,000 to $130,000 , commensurate with experience and qualifications.
