Work Environment

This position is based in an onsite outpatient office in Jacksonville, Florida, where you will collaborate closely with a team of healthcare professionals, including physical therapy assistants and other interdisciplinary team members. You will work Monday through Friday, ensuring a balanced work-life schedule that promotes both professional development and personal well-being.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical capabilities, using diagnostic tests to assess muscle functionality, joint stability, and overall mobility.
• Develop and implement individualized treatment plans tailored to each client's unique needs, aimed at preventing disability, alleviating pain, restoring function, and maximizing performance.
• Oversee the work of physical therapy assistants to ensure they administer treatment as prescribed and according to the plan of care.
• Assess patients' home environments and recommend modifications to improve their safety and independence during rehabilitation.
• Provide hands-on physical therapy services including therapeutic exercises, manual therapy techniques, traction, and the application of physical agents such as heat or cold therapy.
• Fit and adjust prosthetic and orthotic devices, making recommendations for modifications as necessary, in consultation with physicians.
• Consistently monitor and document patient progress, adjusting treatment plans as needed, and communicate any changes to the interdisciplinary team.
• Collaborate in case conferences, contributing to the discussion and progression of individual treatment goals while consulting with other healthcare providers as needed.
• Instruct patients and their families on the proper use of therapy devices and self-management strategies for ongoing recovery.
• Maintain accurate clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring timely submission according to clinic policies.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Valid physical therapy licensure in good standing within the state of Florida.
