Work Environment

This role is based onsite in the Jacksonville area, primarily operating within the home care environment. This setting allows you to establish close relationships with your patients, working in the comfort of their homes where they feel the most at ease. Your day will involve traveling to various locations, allowing for a flexible schedule while maintaining consistent documentation and follow-up care.

Key Responsibilities

• Conduct thorough evaluations of patients, utilizing state-of-the-art assessment tools to identify functional limitations and develop personalized treatment plans tailored to each individual's needs.
• Implement effective treatment protocols based on clinical evaluations, adapting approaches to meet the evolving requirements of patients during their rehabilitation journey.
• Maintain comprehensive and accurate documentation of patient progress, including initial evaluations, ongoing treatment notes, and discharge summaries, to ensure continuity of care and compliance with best practices.
• Engage in continuous communication with other healthcare professionals to coordinate care and modify treatment plans as necessary to optimize patient outcomes.
• Assist patients in understanding their treatment plans, goals, and the nature of their conditions, ensuring they are involved in their recovery process.
• Participate in team meetings and professional development sessions to enhance your skills and contribute ideas for improving service delivery.
• Stay current with industry standards and contribute to maintaining compliance with state, local, and federal regulations governing physical therapy practice.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic surgeries, chronic pain, sports injuries, and general mobility limitations. Each patient presents unique challenges, providing dynamic and fulfilling opportunities to deploy your clinical expertise and interpersonal skills. You will work with diverse age groups, tailoring your approach to meet the age-specific needs and preferences of each individual.

Qualifications

• Possession of a Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Current state licensure as a Physical Therapist (PT) by the Physical Therapy Examining Committee in Florida.
• A minimum of two years of outpatient orthopedic experience is required.
• Current certification in CPR to ensure the highest standards of patient safety.

Preferred Experience

• Experience working in a home care setting is highly desirable.
• Knowledge of telehealth practices and the use of technology to enhance service delivery.
• Familiarity with outcome measure assessments and their application in treatment planning.

Compensation and Benefits
