Key Responsibilities

• Evaluate patients referred by physicians and develop individualized treatment plans tailored to their needs.
• Implement and adjust therapeutic exercises and modalities in the home environment to restore function and mobility.
• Utilize evidence-based practices to maximize patient outcomes and ensure compliance with the physician's plan of care.
• Provide education and consultation to patients and their caregivers about treatment procedures and rehabilitation goals.
• Demonstrate excellence in customer service, responding promptly to any patient inquiries or concerns to enhance their care experience.
• Collaborate with a multidisciplinary team to share insights and strategies that contribute to best patient practices.
• Maintain accurate documentation and meet minimum utilization goals while adhering to company policies and protocols.

Patient Population / Client Focus

In this role, you will work with a diverse group of patients across various specialties, including orthopedic, pediatric, neurological, and sports medicine. You will develop strong relationships with your patients, allowing you to empower them to achieve their functional goals and improve their overall quality of life.

Qualifications

• Graduation from an APTA- or CAPTE-approved physical therapy program.
• A Bachelor's Degree in Physical Therapy.
• State licensure in Physical Therapy, ensuring compliance with local regulations.
• Basic Life Support Certification from the American Heart Association to manage emergency situations effectively.
• Strong verbal and written communication skills to connect with patients and their families.
• Proficient computer skills for documentation and treatment planning.

Preferred Experience

• Prior experience in home care settings, demonstrating familiarity with treatments performed in patients' homes.
