Work Environment

In this role, you will work onsite in the comfort of our patients' homes, allowing for personalized care that directly aligns with each individual's needs and rehabilitation goals. You will be part of a collaborative team environment, working alongside other healthcare professionals to ensure comprehensive care delivery. Your schedule will be Monday to Friday, dedicating 40 hours a week to direct patient care, enhancing your interaction with patients and their families in a meaningful way.

Key Responsibilities

• Conduct assessments to evaluate patients' conditions, designing and implementing tailored treatment plans based on physician referrals and individual goals.
• Utilize innovative and evidence-based therapies, engaging in various modalities and rehabilitation techniques to enhance patients' functional capabilities.
• Maintain thorough documentation of patient progress and outcomes, adhering to the physician's plan of care while adjusting therapies as necessary.
• Provide education and training to patients and their caregivers, empowering them with knowledge about their conditions and treatment options.
• Demonstrate exceptional customer service, ensuring a positive experience for all patients while maintaining effective communication with their families.
• Meet or exceed established utilization goals and contribute to the operational success of the practice.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy.
• Active State License in Physical Therapy in Florida.
• Basic Life Support Certification from the American Heart Association.

• Proficient in word processing and other computer systems relevant to maintaining patient records.
• A desire to create meaningful connections with patients and a commitment to their rehabilitation journey.
• Ability to establish community relationships that enhance the practice's visibility and patient referrals.

Preferred Experience

Prior experience in home health or outpatient settings is a plus, particularly in providing specialized treatment for the populations mentioned. Familiarity with electronic health records (EHR) systems and a background in evidence-based practice approaches will also be beneficial. Compensation and Benefits
