Position Overview

Work Environment

Key Responsibilities

• Conduct thorough assessments to evaluate patients' physical abilities, functional limitations, and overall health status.
• Develop and implement personalized treatment plans that address specific patient needs, including interventions aimed at pain relief, mobility enhancement, and restoring muscle function.
• Administer therapeutic exercises, techniques, and modalities such as manual therapy, traction, and the application of therapeutic agents.
• Monitor and document each patient's progress diligently, adjusting treatment plans as necessary based on patient feedback and clinical findings.
• Oversee the physical therapy assistant's implementation of the treatment plan and offer guidance and support as needed.
• Evaluate the home and rehabilitation environments for accessibility and safety, recommending changes to facilitate better patient outcomes.
• Collaborate with interdisciplinary teams, including physicians and case managers, to coordinate care and share insights regarding patient progress and needs.
• Participate in case conferences and contribute to discussions on best practices and innovative treatment approaches.
• Educate patients and their families on effective strategies for ongoing recovery, including home exercise programs and the proper use of devices.
• Maintain thorough clinical documentation, including initial evaluations, progress reports, and discharge summaries.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from surgeries, injuries, and chronic conditions. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A valid PT license in good standing in the state of Florida.
• Current certification in CPR.
• At least one year of experience as a licensed physical therapist in either acute care or home care settings.
• Strong communication and interpersonal skills, with a passion for enhancing patient care.
• Ability to work collaboratively within a multidisciplinary team.

Preferred Experience

• Experience in developing comprehensive rehab plans for patients with diverse conditions.
• Proficiency in utilizing rehabilitation tools and technologies and keeping up to date with industry best practices.
• Previous experience mentoring or overseeing support staff such as physical therapy assistants.

Compensation and Benefits
