Key Responsibilities\n

• Directly work with clients both remotely and within communities, addressing the symptoms of major mental illness and severe emotional disorders.

• Participate in interdisciplinary team meetings to collaborate on client care and treatment goals.
\n
• Conduct brief, evidence-based screening and employ further assessment techniques to develop a comprehensive view of client needs.

• Create and implement integrated care plans, ensuring that clients have access to the necessary resources and services.
\n
• Facilitate individual, group, and family therapy sessions to support clients in achieving their emotional and mental health objectives.
\n
• Monitor and evaluate client progress, making adjustments to treatment plans as required and ensuring clients receive appropriate services based on their needs.

• Prepare and submit timely reports and documentation that reflect client progress, treatment outcomes, and any necessary organizational data.

• Provide on-call crisis intervention support as needed, ensuring clients in distress receive immediate attention.
\n
• Participate in committees focused on improving agency processes and service delivery, committing to professional development and quality improvement for the agency.

Patient Population / Client Focus\n

This role primarily focuses on individuals within the hospice care spectrum, which entails working with clients and families facing end-of-life challenges. You will navigate complex emotional landscapes and help clients process grief, loss, and acceptance. Your expertise will guide clients to therapeutic resources necessary for navigating their unique situations, ultimately ensuring they experience dignity and comfort during the hospice process.

Qualifications\n

• LCSW licensure in the state of Arizona, with a strong understanding of psychosocial assessment, diagnosis, and treatment aligned with professional standards.

• Expertise in crisis intervention techniques and a solid foundation in therapeutic approaches to mental health care.

• Strong verbal and written communication capabilities, enabling effective interaction with clients, family members, and a multidisciplinary team.

Preferred Experience\n

• A minimum of 2 years of direct clinical experience, preferably within a hospice or mental health setting.

• A Master’s degree in Social Work from an accredited institution is required.

• Proven experience in utilizing electronic health record (EHR) systems for documentation and client management.

Compensation and Benefits\n

The annual salary for this position ranges from USD 70,000 to 85,000, commensurate with experience and qualifications.
