This full-time position offers a unique opportunity to work remotely from the comfort of your home in Bellevue, Washington, while making a positive impact on the lives of our patients served at our Hospice Center. Key Responsibilities\n

Your primary responsibilities as an LICSW will include:

• Delivering effective clinical services to eligible children and families in accordance with best practice standards, utilizing evidence-based treatment methodologies. As part of your onboarding, obtaining CANS certification will be required within two months of hire.
\n
• Engaging in collaborative efforts with schools, social service agencies, and other healthcare providers to ensure a seamless continuum of care. This will involve building teams and utilizing natural support systems that benefit families within the WISe framework.
\n
• Assisting clients in accessing vital resources and education that promote their autonomy and address fundamental needs, thus empowering them to achieve better health outcomes.
\n
• Monitoring assigned clients for Legal Risk Assessments (LRA) compliance. You will also be tasked with facilitating LRA extensions and coordinating with the Crisis Response Team (CRT) and court personnel to maintain ongoing support for clients as needed.

Patient Population / Client Focus\n

The core mission of this role centers around serving patients across various demographics, particularly children and families facing mental health challenges. As part of our Hospice Center's outreach services, you will encounter diverse backgrounds, including working with unhoused populations, requiring sensitivity and respect for individuals’ unique struggles and circumstances. Your contributions will significantly enhance the overall quality of life and support systems for clients who may be navigating complex emotional and social issues.

Qualifications\n

To be successful in this position, candidates must possess the following qualifications:

• A current and valid LICSW license from the state of Washington.

• A Bachelor's Degree in Social Work or a related field, with a strong understanding of the principles related to mental health assessment and treatment.

• Three years of experience in a clinical setting, providing therapy or counseling services, especially within a primary care framework. Specialized training and support will be offered to those who are new to integrating behavioral health.

• Solid understanding of Medicaid, Medicare, and private insurance plans along with the associated navigation processes.
