Key Responsibilities\n

• Establish strategic partnerships with community organizations to enhance our patients' aftercare plans, navigating resources to improve support for families.

• Identify and address areas of resistance and conflict among patients, families, and healthcare team members, employing conflict resolution strategies to facilitate productive discussions.
\n
• Maintain accurate and up-to-date patient records, documenting assessments, treatment plans, and interdisciplinary communications in a timely manner using established formats.
\n
• Apply clinical expertise in evaluating patients using the latest DSM diagnostic criteria, understanding age-related developmental tasks to guide treatment.
\n
• Conduct thorough interviews and consultations with patients and families to formulate relevant psychosocial assessments that inform treatment planning.
\n
• Develop and continually refine personalized treatment plans based on ongoing assessments of patients’ psychosocial needs, strengths, and values.
\n
• Lead and facilitate educational and therapeutic group sessions, documenting patient goals and responses for continuous improvement of therapeutic interventions.
\n
• Participate in treatment-planning meetings, providing valuable feedback and updates to adjust treatment strategies based on patient progress and challenges.

Patient Population / Client Focus\n

This role engages particularly with patients in hospice care, requiring sensitivity to their emotional, social, and spiritual needs as they navigate complex life transitions. You will work closely with families to create supportive environments that honor patients' wishes and provide comfort through tailored interventions.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline.
\n
• Possession of an independent LCSW license is mandatory.
\n
• Basic Life Support (BLS) and CPR certification.
\n
• Strong organizational skills and the ability to manage documentation efficiently.
\n
• Excellent verbal and written communication skills, enabling effective interactions with patients, families, and team members.
\n

Preferred Experience\n

• Experience in hospice or healthcare settings is strongly preferred.

• Familiarity with behavior modification techniques, assertiveness training, and supportive therapy methodologies.

Compensation and Benefits\n

We offer a competitive salary range of $120,000 to $130,000 annually for this full-time position, commensurate with experience and qualifications. In addition to competitive pay, we provide:
