Key Responsibilities\n

• Develop and provide a tentative schedule of appointments, home visits, and observations to supervisors, ensuring alignment with program objectives.

• Coordinate home visits, including chart reviews and staff interviews, to gather critical data for analysis.

• Monitor the effectiveness of treatment plans and make necessary adjustments to enhance patient outcomes.

• Participate in interdisciplinary team meetings for collaborative discussions on best practices and strategies for seamless patient care delivery.

• Document monthly and quarterly progress notes that accurately reflect patient development and treatment adjustments.

• Organize training sessions for clinical staff to enhance competencies in implementing treatment plans and maintaining accurate documentation.

• Provide follow-up services, including interviews and observations, to ensure continuous support for individuals seeking mental health resources.

Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from USD 90,000 to 110,000, depending on experience and caseload availability.
