Key Responsibilities\n

• Conduct thorough assessments and develop personalized treatment plans that address the specific needs and goals of each patient.

• Maintain clear and effective communication with supervisors, team members, family members, and other stakeholders regarding patient progress and care strategies.

• Create a structured schedule that accommodates patient appointments, home visits, and collaborative observations to support continuous assessment and progress monitoring.

• Organize and execute home visits, ensuring they align with program requirements and contribute to comprehensive care.

• Participate actively in Interdisciplinary Team Meetings to collaborate with colleagues on patient care and share insights from your counseling experience.

• Document progress through monthly and quarterly notes, ensuring that all records are up-to-date and accessible to relevant parties.

• Lead training sessions to ensure that staff and team members are competent in implementing program plans and adhering to documentation standards.

• Act as a liaison between the mental health community and the public, promoting awareness and understanding of mental health issues and available resources.

• Assist in the creation and refinement of training materials, aiming to elevate the quality of care provided to individuals.

Patient Population / Client Focus\n

Your primary focus will be on a diverse patient population, which includes children, adolescents, and adults dealing with various mental health concerns. Qualifications\n

• A Master’s degree or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of an active and unrestricted LPC license in the state of Texas.
