Key Responsibilities\n

• Assess and provide mental health support to individuals dealing with various emotional and psychological challenges, while also engaging with family members in the therapeutic process.

• Provide education and counseling to family caregivers, assisting them in understanding their loved ones' conditions and navigating available resources.

• Utilize evidence-based practices and electronic medical records (EMR) to efficiently monitor clients' progress toward their therapeutic goals.

• Participate in care system processes aimed at preventing hospital readmissions, including identifying at-risk patients and facilitating timely interventions.

• Conduct transition planning conferences with patients and families to discuss ongoing care needs and available support services.

Qualifications\n

Compensation and Benefits\n
