This role is fully remote, with the flexibility to work from home while maintaining regular communication with the hospice care facility and other team members. Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify clients' emotional and mental health needs, strengths, and available resources.

• Deliver individual, family, and group therapy sessions utilizing therapeutic modalities such as cognitive-behavioral therapy and trauma-focused therapy.

• Collaboratively develop and implement personalized treatment plans, setting clear goals and interventions tailored to each client.

• Coordinate closely with psychiatrists, psychologists, and the broader treatment team to ensure comprehensive, client-centered care.

• Provide timely crisis intervention, conducting risk assessments and implementing safety plans as necessary.

• Maintain accurate and compliant documentation in line with ethical, legal, and organizational standards.

• Facilitate engaging group therapy sessions and educational classes to enhance client knowledge and coping strategies.

Patient Population / Client Focus\n

Your primary focus will be on clients facing the complexities of hospice care, including their families. This population often grapples with grief, anticipatory loss, and various mental health concerns such as anxiety and depression related to their circumstances. You will play a crucial role in providing emotional support, helping clients navigate their feelings, and facilitating discussions about life transitions with empathy and sensitivity.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Texas.

Compensation and Benefits\n
