Work Environment

This position is primarily onsite, allowing therapists to engage directly with patients in their home settings. You will work a standard 40-hour week, with variable hours that may include weekends, providing flexibility in your schedule while ensuring timely and effective patient care. Key Responsibilities

• Conduct comprehensive assessments of patients with functional limitations, creating tailored treatment plans that address individual needs.
• Utilize evidence-based practices to implement treatment protocols, including therapeutic exercises, manual therapy techniques, and patient education.
• Engage in regular evaluations and re-evaluations of treatment effectiveness, adapting care plans as necessary to achieve optimized outcomes.
• Maintain detailed and accurate patient documentation in compliance with all legal and professional standards, ensuring timely submission of charges for services rendered.
• Collaborate with a variety of healthcare providers, including physicians and specialists, to ensure a comprehensive continuum of care for each patient.
• Lead and participate in community outreach initiatives aimed at promoting wellness and rehabilitation, enhancing Stone Crest's role in the community.
• Actively participate in departmental meetings and ongoing education programs to improve clinical skills and knowledge.

Our Home Care Physical Therapists work with a diverse population of patients, including individuals recovering from surgeries, managing chronic conditions, or overcoming physical disabilities resulting from injuries or illnesses. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure in Physical Therapy and good standing with regulatory boards.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
