Work Environment

You will be working onsite, primarily at the homes of patients across Red Bank and surrounding areas. This dynamic work setting requires adaptability and strong organizational skills to effectively manage your schedule while ensuring that you meet the diverse needs of each patient. Key Responsibilities

• Conduct comprehensive evaluations to assess patients' needs, including reviewing medical histories, performing specific assessments, and determining appropriate treatment plans.
• Effectively communicate evaluation results and treatment strategies to patients and their families, engaging them in the goal-setting process to ensure collaborative care.
• Implement treatment protocols based on individual evaluations, regularly adjusting plans to reflect patients' health progression and responses to interventions.
• Collaborate with a multidisciplinary team of healthcare professionals as necessary, documenting any changes in patients' conditions that warrant modification of the care plan.
• Manage discharge planning to ensure effective continuity of care, documenting all patient interactions and progress in a clear and timely manner according to regulatory standards.
• Strive to meet productivity expectations while managing your time effectively; maintain timely attendance for meetings and patient appointments.
• Contribute to departmental goals and quality improvement initiatives, actively participating in team meetings to enhance service delivery.
• Engage in ongoing professional education and serve as a clinical instructor to students, ensuring a high standard of practice within the organization.
• Promote a positive patient experience by being flexible and responsive to their needs throughout the therapy process.
• Take an active role in initiatives to improve access to care and patient satisfaction metrics.

Patient Population / Client Focus

Your primary focus will involve working with diverse patient populations, including individuals recovering from surgery, those with chronic illnesses, and seniors requiring rehabilitation services. Each patient's care plan will be tailored to their unique conditions and goals, allowing you to directly see the impact of your work on their recovery and overall well-being.

Qualifications

• A Doctorate Degree in Physical Therapy (DPT) from an accredited program is required.
• A valid state license is mandatory before commencing work.
• Certification in Basic Life Support (BLS) for Healthcare Providers is needed.
• If you possess a minimum of 2 years of experience in a relevant physical therapy setting, it will be beneficial, but not required for all applicants.

Preferred Experience

• Familiarity with home care services and working with geriatric patients will be an advantage.
• Experience in developing individualized treatment plans and documenting patient outcomes effectively is preferred.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 per year, commensurate with your experience and qualifications. In addition to a rewarding salary, you will receive a comprehensive benefits package that includes:
