Work Environment

This position requires a hands-on approach as services are provided onsite in patients' homes. You will receive support from administrative staff, allowing you to focus primarily on patient care. Key Responsibilities

• Conduct comprehensive evaluations of patients in a home care setting, assessing physical performance and functional limitations resulting from injury, disease, or surgical procedures.
• Create detailed, individualized treatment plans based on assessment findings, incorporating various therapeutic modalities including exercise, manual therapy, and patient education.
• Implement and adjust treatment plans as necessary, documenting patient progress and response to therapies accurately and thoroughly.
• Reevaluate patients periodically to determine effectiveness and make recommended changes to treatment strategies as needed.
• Educate patients, families, and caregivers on treatment protocols, assistive devices, and rehabilitation processes to ensure optimal care and understanding of therapeutic goals.
• Engage in ongoing education to keep abreast of the latest physical therapy techniques and methodologies through workshops, seminars, and professional networks.

Patient Population / Client Focus

Your patients will range from children to seniors, each with diverse needs, encompassing post-operative rehabilitation, management of chronic conditions, and developmental disabilities among others. Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Must hold a valid Physical Therapy license in the state of New Jersey.

• Flexibility in adapting to various patient needs and environments.

Preferred Experience
