Work Environment

This full-time position is primarily onsite, allowing you to provide individualized therapy sessions in the comfort of patients' homes. Our home care model facilitates a comprehensive approach to therapy, ensuring personalized attention and care tailored to each patient's unique circumstances. The typical work schedule is Monday through Friday, from 9 am to 5 pm, which allows for a work-life balance while delivering critical services to those in need.

Key Responsibilities

• Conduct thorough evaluations of patients to formulate effective treatment plans that address their specific objectives and conditions.
• Manage a caseload of one patient per hour, ensuring high-quality care and uninterrupted focus on their rehabilitation.
• Utilize evidence-based practices and manual therapy techniques to deliver cost-effective treatments that restore mobility and improve physical function.
• Collaborate with a multidisciplinary healthcare team to enhance patient outcomes through integrated care approaches.
• Monitor and assess patient progress regularly, adjusting treatment plans as needed to achieve optimal results.
• Maintain precise and confidential documentation for patient evaluations, treatment progress, billing, and regulatory compliance.
• Participate in ongoing education and training to stay current with industry standards and emerging practices in physical therapy.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will primarily serve a diverse population of patients who require rehabilitation following surgeries, orthopedic injuries, and chronic conditions. Your expertise will play a crucial role in helping patients regain independence and improve their quality of life through targeted interventions designed to meet their individual needs.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid New Jersey physical therapy license, with a clean record free of disciplinary actions.
• A minimum of 2 years of experience providing physical therapy services, preferably in an outpatient or home care setting.
• Proficient in manual therapy techniques and able to implement innovative treatment strategies.
• Demonstrated ability to work independently while enjoying collaboration within a team environment.

Preferred Experience

While the required experience is 2 years in physical therapy, candidates with additional experience in orthopedic outpatient care and expertise in specialized therapy techniques are preferred. Familiarity with home care dynamics and an understanding of patient-centric care models will enhance your ability to succeed in this role.
