This position is based onsite, primarily focusing on home care services in Union City, NJ. Key Responsibilities

• Conduct comprehensive assessments of patients to evaluate their physical abilities, limitations, and specific rehabilitation needs.
• Develop individual treatment plans tailored to each patient's goals, preferences, and medical history, regularly adapting strategies based on progress.
• Implement therapeutic interventions such as exercise programs, manual therapy techniques, and patient education to optimize recovery.
• Collaborate with an interdisciplinary treatment team, including physicians and support staff, providing detailed documentation regarding patient treatment regimens and progress.
• Provide ongoing support and education to patients and their families, ensuring understanding of treatment goals and at-home exercises.
• Adhere to all applicable laws, regulations, and professional standards of care while maintaining accurate records of treatments and evaluations.
• Participate in quality improvement initiatives, community outreach, and marketing efforts as needed, alongside supervising support staff.
• Engage in continuous professional development through self-study, attending workshops, and pursuing further training opportunities.

Patient Population / Client Focus

Your work will primarily focus on patients recovering from orthopedic surgeries, sports injuries, neurological conditions, and age-related physical decline. By customizing your approach to address each patient's unique challenges and aspirations, you will play a vital role in their journey toward regaining physical independence and enhancing their overall well-being.

Qualifications

• Possession of a Bachelor's Degree in Physical Therapy is required.
• A valid State License in Physical Therapy must be maintained in good standing.
• Basic Life Support (BLS) certification must be obtained and kept current.
• Exceptional verbal and written communication skills are essential for effective patient interaction and documentation.
• Proficiency in word processing and electronic health record systems is required to document patient interactions accurately.

Preferred Experience

While a minimum of two years of experience as a Physical Therapist in a clinical or home care setting is required, candidates with a background in (or familiarity with) geriatric care and advanced rehabilitation techniques will be viewed favorably. Experience supervising support staff is also a plus.

Compensation and Benefits

Mindful Physical Therapy offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and expertise.
